NYC Health + Hospitals has built one of the largest health system-based community health worker programs in the nation with over 250 new community health workers.

NYC Health + Hospitals today announced that more than 5,000 patients have worked closely with community health workers in the program’s first year. Community health workers at NYC Health + Hospitals serve adults with special needs such as primary care adult patients, families with young children, homelessness, involvement in the criminal justice system, and behavioral health conditions. increase.
- Adult Primary Care Program: Community Health Workers are part of the primary care team at all 11 NYC Health + Hospitals hospitals and six Gotham Health sites. 1,218 adult patients graduated from the 3-month Adult Primary Care Community Health Worker Program. Community health workers primarily assisted patients with gaps in food insecurity, financial insecurity, medical costs, health insurance, transportation, housing insecurity, social support, and legal needs. Nearly all patients in the program have one or more chronic conditions. The most common diagnoses for program patients are hypertension, diabetes, heart disease, cancer, asthma, chronic kidney disease, and chronic obstructive pulmonary disease (COPD).
- behavioral health program: Approximately 40 behavioral medicine community health workers serve adult patients with mental health screenings, substance use disorders, or both. They have served approximately 2,100 patients. Almost half of these encounters focused on aftercare follow-up and access to care services, and the remaining quarter focused on housing assistance, access to benefits services, education or employment services, legal and is dedicated to connecting patients to specific, specific services, such as immigration needs.
